The PLUS Company is dedicated to improving advocacy and community efforts that
promote nurturing and healthy living environments for elderly individuals. Through family care or individual care, we assist consumers in making choices about their own lives. Our highly qualified staff provides assistance and supervision in
designing the supports and services needed to allow an Elder to reside in a community home.
The Family Home Care project focuses on the need for quality alternative living situations and choices for seniors needing mid level care.
This program collaborates PLUS team members with caring members in our community who provide a home to our consumers. The benefits for such a provider are not only a monetary stipend, but special relationships are created that enrich the lives of all involved. Caring and unique people make this program possible.
Adult Family Care is a combination of personal care, homemaking and other services provided to a person in a licensed home.
Individual Care develops and directs the types of care provided which could include, but are not limited to:
- Reliable supervision and case management;
- Ensure safety and safety awareness;
- Medical management (including supervision of medication and health care needs);
- Health and nutrition;
- Arranging appointments and transportation;
- Assistance with activities in daily living and self advocacy;
- Housekeeping, personal hygiene and laundry service;
- Access to community programs and activities;
- Overnight assistance.
The PLUS Company's certified nurses and home health aides have a strong presence in overseeing the program. A residential coordinator oversees and monitors the licensed home.
